Description/Job SummaryThis position is located in Rapid City area. This is a professional level position that will have the ability to work from home. However, in-state travel is required. The overall responsibility of this position will be administering the CHOICES waiver and CTS program, and coordinating services for adults and children who have an intellectual or developmental disability. This includes, but is not limited to, coordinating services to meet medical, psychiatric, social, nutritional, educational, vocational, employment, housing, transportation, recreational, legal, and advocacy needs. Case Managers will provide support to individuals to actively engage in and direct the person centered plan development process.Case Manager will lead person centered planning efforts, focusing on what is /important to/ and /important for/ those on their case load, and designing the supports that will be most effective in achieving a balance between the two. Case Managers will advocate for those on their caseload, assisting with decision making about the supports and services they want and need, and facilitating the provider selection process.Compensation includes a competitive hourly rate, mileage reimbursement, and initial cost of home office set up.Responsibilities/DutiesEssential Duties and Functions

  • Facilitate development of ISPo Facilitate pre-meetingo Work with each participant to invite team members for in=person meetingo Conduct and coordinate assessment completiono Write the Standardized ISPo Assure implementation of ISPo Assume lead over Person Centered Planning efforts, including utilization of discovery and PCT tools in accordance with HCBS regulationso Facilitate annual ISPo Facilitate team meetings as changes are needed or as required.
    • Ongoing monitoring of ISPo Complete all components of ISP monitoring quarterly, including face to face contactEnsure follow up from Community Support Providero Submit monitoring to the support teamo Conduct monitoring in multiple settings/locations for a participant where supports are being provided
      • Critical Incident Reportso Review Community Support Provider internal incident reportso Identify Critical Incident Reports and report to Division of Developmental Disabilitieso Investigate Abuse, Neglect, and Exploitation reports when related to allegations involving RHD Case Managemento Reporting responsibilities to families and guardianso Follow up on Critical Incident Reports and facilitate changes to plan if needed (preventative measures)o Ensure follow up by RHD and Community Support Provider
        • Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC)o Review approval prior to implementation and annually thereaftero Participate in HRC/BIC meetingso Assist as needed with completing documentation, and submitting to HRC/BICo Review recommendations from HRC/BIC and ensure are included in plano Assist in obtaining consents for psychotropic medications when time permitso Identify emergency restrictions, and document and report to HRC/BIC and team within 24 hours
          • Othero Focus on Personal Outcome measureso Knowledge and understanding of Charting the Life Course frameworko Evaluate the needs of those on caseload in order to remain successful in the communityo Link those on caseload to programs and supports that are needed and requested to meet their needso Maintain caseload of individuals with intellectual or developmental disabilitieso Complete required documentation of services, including input of data, reports, and plans into TherapRequired ExperienceQualified candidates will possess a bachelor s degree or equivalent in human services, social work, sociology, or psychology, and 2 years of experience working in a similar role.
