A Community Health Worker is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. -American Public Health Association 2020

The Community Health Worker will will primarily work Monday - Friday 8am-5pm, but may have some evening/weekend needs to meet the needs of the patients. A Community Health Worker will also be required to complete community education classes weekly so these will most likely take place in the evenings.

This is creative and rewarding work helping to find resources for community members.

Job Summary

The Associate Community Health Worker (CHW) will serve as a bridge between under-served communities and the health and social service systems. CHWs are defined by the American Public Health Association as "a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of the service delivery." As a non-clinical role, the CHW works within the patient-centered medical home model, which embraces a team approach to chronic disease self-management. Provides information and assistance to help patients navigate a complex health and human services system. Strong advocacy and raising awareness of individual and community health needs are necessary for efficient and culturally sensitive work. Under indirect supervision, this individual would work closely with medical providers, primary care teams, and social services agencies to provide care coordination and connection to resources and support to program clients to improve their health and general well-being through education and provision of coordination of care and services. This individual would work in both clinical and community-based settings.

Must strive to build capacity for access to services and community engagement. Strong communication skills from a non-judgmental perspective will be necessary to engage patients in their own care, articulate clearly and effectively when advocating for patients and community groups, and assist members in understanding and effectively navigating complex health systems. Effective active listening skills are essential. Actively engages in relationship building and work from a team-based perspective.

Collaborates on goal setting and care planning within an inter-professional team context. Required to give presentations to community groups, and requires strong public speaking and presentation skills that include diplomacy and tact. Strong organization skills and the ability to anticipate future planning needs is required. Effective coordination skills are necessary in facilitating appointment attendance and individual plans of care. Serves as a point person for issues and related solutions while maintaining appropriate boundaries.

Holds knowledge and experience of ethnic and cultural populations, government partners, community stakeholders, and other organizations. Models effective problem solving skills to empower patients to take an active role in their health care. Must have a thorough understanding of self-efficacy theory and its application when working with others who have chronic disease. Embraces cultural sensitivity when seeking information and working with others.

Documents completion of assigned activities and support of community members as required. Provides services under supervision as directed by state or other regulatory requirements. Strictly adheres to the scope of their duties and will not give medical advice under any circumstance. Strong adherence to all patient privacy laws in health care settings and the community is essential.


High school diploma or equivalent preferred.

Knowledge of community services and navigating social support systems preferred. Volunteer experience with local community organizations preferred.

Motivational Interviewing skills are required with training, to be completed within six months of hire.

A minimum of two years' direct experience is required. Must be at least 21 years of age.

Demonstrated ability to communicate effectively verbally and in writing with people of differing cultural and socio-economic backgrounds.

Demonstrated computer skills, including proficiency in MS Office Suite (Outlook, Word, Excel, etc.).

Protect the privacy and security of protected health information as defined by State and Federal law.

Proof of certificate from a Community Health Worker program is required upon hire or within six months.

Become a Better Choice/Better Health Instructor within one year of hire.

Must have a valid driver s license.
