  Job Summary
  
  The Social Worker serves as an important component of the clinical care teams. He/she is -responsible for the planning, organization and implementation of social work services to patients and families in the clinic setting, assuring that medically related emotional and social needs are addressed on an individual basis for the patient. - The Social Worker is -responsible to assess needs of patients facing chronic illness and facilitate plans with patient/family to promote effective care. - He/she will maintain a current knowledge base of available community agencies and resources to facilitate appropriate referrals. -
  
  Education and/or Experience
  
  
  • 
    
  • Master's degree in Social Work from an accredited Social Work Program required.
    
    
    Certification/Licensure/Registration
    
    
    • 
      
    • Current Social Work license in the state of South Dakota required.
      
    • Current Social Work license in the state of Iowa, Nebraska, Minnesota or North Dakota may be required depending on the service area. - If required, must obtain within 90 days of being placed in the position or of the requirement.
      
    • This position requires a valid driver's license and that the employee is insurable by the Avera Health automobile liability insurance carrier.
      
      
      Pay
      
      The hourly range for this position is listed below. Actual hourly rate dependent upon experience. -
