Job Description:
A company is looking for a Care Manager Transition of Care.
Key Responsibilities
Evaluate member needs through post-discharge assessments and
medication reconciliation
Develop care/service plans and collaborate with healthcare teams to
support member transitions
Facilitate education and resources for members and caregivers to
enhance wellness and care quality
Required Qualifications
Degree from an Accredited School of Nursing
2 - 4 years of related experience
RN license required
Bilingual preferred
Must reside in WA
