eligible applicants Department Details Ava’s House is a 20-bed
inpatient acute hospice facility. We care for adult and pediatric
patients and have cared for patients ranging from the age of one
day old to over 100 years old. We offer a collaborative
interdisciplinary team that promotes engagement and education. Our
interdisciplinary team is made up of physicians, nurse
practitioners, nurses, patient care techs, social workers,
pharmacists, volunteer representatives, and chaplains. Our facility
focuses on symptom management and maintaining comfort and dignity
at end of life. We provide 24-hour care and could be considered a
“Hospice ICU”. We take deep pride in our ability to manage
distressing symptoms in medically and ethically appropriate ways.
We understand that this is a very difficult time for our patients
and their families, and we work tirelessly to meet them where they
are on their journey. Our department works closely with all
inpatient units, especially medical oncology. Medical oncology
staff care for our patients until they are able to be transferred
to Ava's House. Job Summary The Registered Nurse in the Clinical
Care Leader (CCL) role facilitates health care services for
patients in cooperation with the health care team. Facilitates
communication and provision of information and messages in a way
that produces clarity and impact. Understands the importance of
inter-team collaboration in breaking down silos and achieving
business results and the ability to lead employees from various
functions to communicate, coordinate work across divisions, and
collaborate in solving problems as one team. Applies the concept of
collaboration to build relationships and coordinate with other
teams. Provides feedback to members of the health care team.
Responsible for utilizing the nursing process (assessment,
diagnosis, outcomes/planning, implementation and evaluation) to
provide individualized nursing care to patients. Collaborates with
other inter-professional colleagues, including physicians, to plan,
implement and evaluate care. Demonstrates competency and practices
within the full scope of nursing expertise/knowledge and utilizes
appropriate age and population specific standards as designated in
their assigned clinical setting. Cares for patients in all phases
of preventative care, health maintenance, diagnosis, treatment, and
follow-up as patients move along the continuum of care. Responsible
for the coordination of care, patient assessment, patient
education, triage, and various other nursing interventions.
Functions within the scopes and standards of nursing practice as
outlined in the Nurse Practice Act and Administrative Rules in
state of practice and licensure. The Sanford Professional Nursing
Practice recognizes the Scope and Standards of Practice and the
Code of Ethics for Nurses with Interpretive Statements as published
by the American Nurses Association as the foundation of nursing
care delivery and professional conduct. Competent in the registered
nurse (RN) skills of the unit/department. Knowledge of procedures
and programs to ensure employee safety. Assumes leadership role in
high-risk situations. Coordinates the clinical/unit nursing
activities while adhering to Sanford Health philosophy, goals, and
standards of care. Acknowledges their impact on budget and finance
through staffing and ensuring safe patient care. The CCL uses
critical thinking skills to collaborate and coordinate patient care
and manage emergent situations, for the specific assigned clinical
setting. Responsible to intervene when unsafe practices are
identified. Responsible to make patient assignments based on
employee's competencies, scope of practice, experience and
patient's unique needs. Demonstrates effective communication skills
to negotiate and resolve conflicts with and between peers,
inter-professional colleagues, patients and families. Serves as a
mentor to staff while facilitating the over-all unit activities.
Participates in house-wide and/or departmental committees and
assists with the coordination of the Peer Review Process for
employees. Participates in active learning and development
resources. The nurse has knowledge of and utilizes appropriate
age-related patient care protocols relating to the physical and
psychological needs of patients according to department scope of
service. Qualifications Bachelor's degree in Nursing strongly
preferred in Fargo, Fargo Network, Bemidji, Sioux Falls Network and
Rapid City. Required in Bismarck and Sioux Falls (Sanford USD
Medical Center and Clinics). Graduate from an accredited nursing
program, including, but not limited to, those accredited by the
Commission on Collegiate Nursing Education (CCNE), Accreditation
Commission for Education in Nursing (ACEN), or National League for
Nursing Commission for Nursing Education Accreditation (NLN CNEA).
Minimum of two years’ clinical experience preferred. Currently
holds an unencumbered RN license with the State Board of Nursing
where the practice of nursing is occurring and/or possess
multistate licensure if in a Nurse Licensure Compact (NLC) state.
Obtains and subsequently maintains required department specific
competencies and certifications. Certification may be required
depending on specialty or service area. Basic Life Support (BLS)
required within 60 days of employment into position. Benefits
