This role is designed for professionals dedicated to facilitating healthcare with dignity and respect, particularly within underserved populations. You will travel locally to administer daily medications and oversee the health and wellness of patients residing in various long-term care facilities, ensuring compliance with prescribed healthcare plans. Travel to various long-term care facilities within the community to provide medication management and support. Administer prescribed medications to patients according to the healthcare provider's instructions and document all interactions. Observe and report any changes in patients' conditions and effectiveness of medications or treatments. Collaborate with healthcare professionals to plan and evaluate healthcare interventions. Educate patients and families on medication therapy and general wellness practices. Ensure storage and disposal of medications comply with current medical and safety standards. Proficient in managing medication therapies for diverse patient populations. Strong understanding of medication administration protocols and compliance. Excellent communication and interpersonal skills. Ability to work autonomously and make decisions in critical situations.
