Job Description:

who is proficient in delivering long-term care. The ideal candidate
will support our mission by providing compassionate and

difference in the lives of those who need it most. Provide direct
patient care under the supervision of nursing staff Record and
monitor patient’s condition, including vital signs, food and liquid
intake, and other necessary information Assist patients with daily
activities such as dressing, bathing, eating, and mobility Work
collaboratively with the nursing team to plan, implement, and
evaluate patient care plans Maintain a safe and clean environment
for all patients Communicate effectively with patients and their
families, providing emotional support Administer medications as
prescribed and observe reactions or side effects Report any changes
in patients' health status to relevant nursing staff immediately
Strong interpersonal and communication skills Ability to work
collaboratively in a healthcare team Detail-oriented with a strong
emphasis on patient safety Excellent physical stamina and ability
to assist patients with physical activities Proficient in
maintaining and updating patient records Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Certification as a
Nursing Assistant (CNA) CPR and First Aid certification Completion
of a state-approved education program
