Job Description:
Good Samaritan Health Center seeks a dedicated and
detail-oriented Minimum Data Set (MDS) Nurse to join our team,
focusing on delivering quality healthcare to our underserved
communities. In this role, you will be responsible for completing
accurate assessments and data collection according to federal and
state regulations to aid in the development and evaluation of
individualized care plans for our patients. This position requires
a compassionate individual who is committed to ensuring holistic
health and wellness outcomes for all patients. Conduct
comprehensive assessments and periodic reassessments of patients as
required by state laws and regulations. Collaborate with a
multidisciplinary team to develop and manage individualized care
plans for each patient. Ensure accurate and timely completion of
the MDS assessments and RAI process to meet regulatory compliance.
Participate in patient care conferences and provide guidance based
on MDS outcomes. Assist with the training and education of the
nursing staff concerning the MDS process. Monitor and ensure that
documentation in the patients’ medical records is complete and
accurate. Advocate for residents' rights and optimal care through
compliant and ethical practice. Proficiency in MDS 3.0 RAI process
Strong analytical and critical thinking skills Excellent
communication and interpersonal skills Detailed knowledge of state
and federal regulations concerning long-term care Capability to
work collaboratively in a multidisciplinary health team Medical
Insurance Dental Insurance Vision Insurance Life Insurance 401k
Current RN license in the state of practice Certified MDS
Coordinator (CMDC) preferred
