listed below. Actual pay rate dependent upon experience. $32.50 -
$48.50 Position Highlights May be eligible for a $15,000 sign on
bonus 1-3 years working in a clinical environment. 8am-8:30pm,
weekend - every 4th, holiday 2-3 per year, minimal call time. Join

Work where you matter. A Brief Overview The Registered Nurse (RN) –
Hospital at Home provides acute-level nursing care to patients
receiving hospital services in their homes. This role delivers
comprehensive, evidence-based clinical care consistent with
inpatient standards while promoting patient safety, comfort, and
recovery in the home setting. The RN functions as part of an
interdisciplinary care team and is responsible for assessment,
clinical intervention, care coordination, and patient education
within the Hospital at Home model. What you will do - Provides
patient care by reviewing physician prescriptions; communicating
the nursing care plan and all pertinent information to other
members of the health care team; coordinating with other healthcare
providers; providing physical, emotional, psychological, and
spiritual support to patients, families, and friends; conferring
with physicians. /n - Functions as interventionalists in the home
utilizing clinical expertise and problem solving to make
recommendations to the physicians based on comprehensive physical,
psychosocial, emotional, spiritual and bereavement patient
assessments. /n - Develops nursing care plans by obtaining medical
histories; examining patients; documenting patient physical
condition, psychological status, and influencing social factors;
Draw labs; Provide respiratory care; conferring with family and
friends; developing a nursing diagnosis; establishing nursing care
objectives; defining nursing interventions, schedules, and
timetables. /n - Teaches healthcare requirements by establishing
patient care goals; instructing patient, family, and friends to
understand condition, medications, and self-care skills; answering
questions; providing information and resources. /n - Evaluates
patient care by adhering to therapeutic standards; measuring health
outcomes against patient care standards; reviewing patient records;
noting changing conditions and needs; re-evaluating care
objectives; modifying nursing care plan. /n - Maintains safe,
secure, and healthy patient care environment by adhering to
infection-control policies and protocols, medication administration
and storage procedures, and controlled substance regulations;
adhering to organization standards and policies, and to legal
regulations; alerting others regarding potential concerns. /n -
Documents patient care services by charting in patient and
department records. /n - Keeps patient care equipment operating by
following operating instructions and established procedures;
troubleshooting breakdowns; maintaining supplies; performing
preventive maintenance. /n - Keeps patient care supplies ready by
checking stock; anticipating needs; placing and expediting orders;
verifying receipt; stocking items. /n - Maintains competence in
nursing capabilities outlined by leadership. Essential

License/Certification, or Work Experience: - Registered Nurse (RN)
- Board of Nursing An active license in the state of practice Upon
Hire Preferred Education, License/Certification, or Work
Experience: - 1-3 years Med Surg, ER, or home health Expectations
